In accordance with New Jersey state law (N.J.S.A. 47:1A-5(f)), which was updated as of July 3, 2023, all government agencies, including the City of Perth Amboy, are required to adopt the Government Records Council’s (GRC) model OPRA request form. This updated form ensures transparency and consistency in how public records are requested and made available.
The City of Perth Amboy is pleased to introduce our agency-specific version of the new OPRA request form, which can now be accessed and downloaded below. If you wish to submit an OPRA request for government records, this form must be used for all submissions moving forward.

How to Submit Your OPRA Request:

- By Mail: Print and mail the completed form to the City Clerk’s Office at 260 High Street, Perth Amboy, NJ 08861
- In Person: Submit the form in person at City Hall during normal business hours of 9 AM - 5 PM EST.
- Electronically: Email the completed form to the Custodian of Records at [email protected].
