INSTRUCTIONS FOR REQUESTING BIRTH, MARRIAGE, DEATH, DOMESTIC PARTNERSHIP OR CIVIL UNION CERTIFICATE IN PERSON OR BY MAIL:
Birth Certificate: Must complete application, or submit signed letter containing name, date of birth, birth name of legal father/parent, and birth name of legal mother/parent.
Marriage Certificate: Must complete application, or submit signed letter containing full birth name of husband, full birth name of wife (before marriage), and date of marriage.
Domestic Partnership and/or Civil Union Certificate: Must complete application, or submit signed letter containing name of partner A, partner B, and date of event.
Death Certificate: Must complete application, or submit signed letter containing name of deceased, place of death, and date of death.
WHO MAY REQUEST YOUR BIRTH, MARRIAGE, DEATH, DOMESTIC PARTNERSHIP, AND/OR CIVIL UNION CERTIFICATE:

Definition of Valid Photo Identification: Legal document showing a photograph of the bearer, and must contain the following: signature, address, and birth date of the bearer. The ID must be issued by a government agency or organization. Ex: Photo Driver Licens

*FEE: $15.00 birth/marriage (records) certificate in the form of money order, payable to the City of Perth Amboy. Death records at $10.00. For any Perth Amboy residents’ age 70 or older: the fee for one certificate per year will be waived. (Mail Applicants: Attach Stamped, Self-Addressed Envelope).
